Rehabilitation service overview

A National Institute for Health and Care Excellence (NICE) concordant, sustainable and recovery-orientated service that supports individuals holistically. Focussed on function improvement rather than symptoms, placing the individual at the centre of their journey with the aim of independent living and mental health rehabilitation.

We work with individuals with severe and enduring mental illness to build a meaningful, independent and satisfying life. 

The care we provide follows interventions in line with NICE guidance (2020):

  • Individuals will direct their own recovery journey through partnership care planning, risk management planning, positive behavioural support plans, recovery stars, occupational therapy plans, medication management plans, psychology formulation and wellness recovery action plans (WRAP) 
  • Individuals will be offered specialised support from our health and wellbeing team
  • Psychological formulation and interventions will be offered to support any unmet psychological needs
  • Individuals will have a comprehensive occupational therapy, social work, nursing and medical assessment as appropriate 
  • Individuals will learn about their mental health to gain insight and understanding of their illness to improve their ongoing recovery during their rehabilitation programme and beyond.

 

Rehabilitation pathway

The rehabilitation pathway consists of inpatient, community and outreach support to individuals.

Individuals can enter the rehabilitation pathway at any stage. They may move up and down the pathway as their needs change. 

Regardless of where individuals are within the pathway, they can access facilities at Grimsby's Harrison House and Hope Court as and when available and appropriate. The principles of the Mental Capacity Act 2005 will be followed at all stages.

The rehabilitation service is registered with the Care Quality Commission (CQC) and adheres to the quality standards developed by the Royal College of Psychiatrists for Inpatient Mental Health Rehabilitation Services (2020).
