The team discuss discharge from the early stages of admission and continue to discuss discharge during MDT meetings and nursing reviews, ensuring that individuals remain focused on recovery and independence.
Discharge does not mean that support ends.
An appropriate plan will be formulated with ongoing support from the rehabilitation service or community services.
Most individuals step down to a placement at Hope Court upon discharge from Brocklesby Lodge.
Hope Court
The prospect of discharge will be discussed early on following admission to Hope Court.
When appropriate, individuals will be supported into secure, suitable accommodation and move on to independent living.
Support from the community rehabilitation team will continue for some time until either a transfer to community services or a discharge from services is agreed.
