Rehab interventions are implemented on a needs basis, and may include:

- Dialectical behaviour therapy (DBT) skills

- Risk assessment (inc. HCR-20, PCL-R, RSVP)
- Psychology
- Mental Capacity Act

Outcome measures
To ensure that an individual’s needs are being met by their rehabilitation plan, the below outcomes are used, as appropriate to the individual.
Those outcomes accepted by NHS England are denoted with an asterisk*:
- Mental Health Recovery Star
- Patient Recorded Experience Measures (PREMs)
- Patient Recorded Outcome Measures (PROMs)*
- General Anxiety Disorder Questionnaire (GAD-7)*
- Therapy Outcome Measures (TOMs)
- World Health Organisation Quality of Life Measure (WHOQoL)
Assessments and reviews
Other assessments are tailored to an individual’s needs and include:
- Positive Behavioural Support Plan (PBS)
- DICES risk assessment and risk management plans
- Collaborative Assessment and Management of Suicidality (CAMS)
- Historical and Clinical Risk Management (HCR-20)
- Occupational therapy plans
Individuals will also be invited to regular MDTs and reviews, including My Care and Support Planning and medication reviews.
