Slow-stream rehabilitation services
Longer-term supported living for individuals requiring ongoing 24-hour care.
Slow-stream rehabilitation focuses more on delivering person-centred care, maintaining an optimum level of independence and quality of life, rather than working towards complete independence.
Within these services, the Mental Health Act and Mental Capacity Act can be utilised to manage risk in the least restrictive environment.
Hub and Spoke
Hub and Spoke allows individuals to live independently in their own flat, whilst still accessing support from the community rehabilitation team, according to the individual’s assessed needs.
Individuals hold a tenancy with a local housing provider and will be supported to maintain this level of independence, building on their skills where appropriate.
If an individual is not yet ready or able to hold a tenancy in their own name, there may be scope for a licence agreement to be held by the Community Rehabilitation Service with a view to transitioning to an assured short-hold tenancy in the individual’s name in the future.
