Referrals and assessments
Referrals to the rehabilitation service are made via SystmOne or by sending a referral form to navigo.rehabreferrals@nhs.net.
A suitability assessment will then be arranged. Once a referral has been accepted, a multidisciplinary team (MDT) rehabilitation needs assessment will be completed.
MDT assessments have the input of nursing, social work and occupational therapy.
Following assessment, the individual and referrer will be notified of the outcome along with the rationale.

Admissions
Admissions to the rehabilitation service take place between Monday and Friday, 9am - 5pm.
Admission to the rehabilitation service ensures a comprehensive assessment of needs and outcomes.
Following a successful referral to the Community Rehabilitation Service, individuals will be allocated a lead support worker to support them through the moving-in process.
Once admitted to an area of the rehabilitation service, the therapy team will formulate a person-centred therapy plan to identify an individual's goals and aspirations which the individual will be supported to achieve.
Care coordination will also be transferred over when appropriate.
