Please complete the following form to order a prescription for your young person.
This facility can only be accepted for young people being treated by our CAMHS ADHD service.
While we try to fulfill the prescription requests as soon as possible, please allow 14 days for your request to be completed. Please do not make a request if you have more than 14 days of medication or this may be rejected.
Once the request has been accepted you will be telephoned to confirm the prescription is ready for collection from Freshney Green Primary Care Centre.

Please note: due to clinic provision, we do not have the facility for prescriptions to be issued on a Monday.
